  • Annual Report Form — As part of their chapter agreement with ASHE, Chapters submit an annual report. The annual report form opens in early December and is due on January 31.

  • Chapter Awards and Application — The ASHE Chapter Awards recognize the outstanding achievements and contributions of its chapters that partner with ASHE to help optimize the health care physical environment. The Chapter Awards application period opens in January and is due on April 1.

  • Chapter Challenge — The Energy to Care Chapter Challenge program allows ASHE affiliated chapters to participate in a friendly competition against one another in an effort to reduce their energy consumption. Chapters who participate in the Chapter Challenge qualify to apply for Elite status during the Chapter Awards season.

  • Chapter Contacts Update Form — Please help us keep our records up to date and complete the Chapter Contacts Update Form if your Chapter has had any leadership, staff support, or general information changes. This information will be used to update our database, the Chapter Section on the ASHE website, and Chapter communications.

  • Chapter Speaker Requests — Chapters can take advantage of the opportunity to have your ASHE Advisory Board Member, the ASHE President, or the ASHE Executive Director present an ASHE update at your chapter’s event free of charge as one of the many benefits of being an ASHE Chapter.

    Chapters who participated in the Chapter Awards program may have one professional staff presentation at any chapter event during the year of their award. Visit the ASHE Speaker Information webpage to learn more about available topics and speakers.

  • Emerging Regional Leader Award — This award recognizes 10 exceptional Emerging Leaders, one from each ASHE region, for their demonstrated commitment to the field and their interest in developing further leadership skills. 

  • Membership List Request Form — Chapters can request an ASHE membership list for their state to utilize for the sole purpose of identifying potential new members and/or business partners within the local/state chapter area.

  • ASHE Member Dues Voucher Request Form — Chapters are awarded vouchers to give out to their members as part of the Chapter Awards Program. The ASHE Member Dues voucher gives a new member one free year of ASHE membership. Chapters select their recipients and submit them by April 1.

  • ASHE Publications Request Form (Platinum/Platinum Elite and Gold Chapters Only) — Chapters are awarded vouchers to give out to their members as part of the Chapter Awards Program. The Publications voucher gives a recipient one of ASHE’s publications at no charge. Chapters select their recipients and submit them by April 1.

  • CHFM and CHC Exam Fee Voucher Request Form — Chapters are awarded vouchers to give out to their members as part of the Chapter Awards Program. The CHFM/CHC exam fee voucher allows a first-time exam taker to take the exam for free. Chapters select their recipients and submit them by April 1.

  • Health Care Facilities Innovation Conference Registration Fee Voucher Request Form — Chapters are awarded vouchers to give out to their members as part of the Chapter Awards Program. The Health Care Facilities Innovation Conference Registration Fee voucher waives the registration fee for the recipient. Chapters select their recipients and submit them via the form by April 1.
