Hospital CMS Tag Checklists and Crosswalks
These crosswalks allow facility professionals to understand how the accreditation standards align with the CMS Conditions of Participation (CoP’s) and Code of Federal Regulations (CFR) and how these align with the adopted codes and standards. The crosswalks provide a comparison of the regulations applicable to most hospitals. This will allow facility professionals to better understand the standards and requirements and thus better prepare for survey evaluations within the physical environment. Additionally, using these crosswalks as checklists will assist facility professionals in developing a safe and healing physical environment for the care of patients.

The ASHE CMS State Operations Manual Appendix A Crosswalk provides A-Tag information along with the related codes and standards applicable to the requirements for each A-Tag. To ensure that the tool is relevant to health care facility professionals, it focuses specifically on A-Tag requirements that affect the physical environment.
This tool will help to ensure that ASHE members understand what is required as they prepare for surveys by accrediting organizations. It also breaks down the requirements in a way that is both accurate and easy to understand, so that health facility professionals can ensure a compliant and safe environment for patients, visitors and staff.
Hospital Emergency Preparedness Survey Readiness Checklist and Crosswalk
ASHE's "Hospital Emergency Preparedness Survey Readiness Checklist and Crosswalk" tool provides the E-Tag information along with the codes and standards applicable to the requirements and how to meet the requirements for each tag. This not only helps to prepare for emergencies, but also ensures a compliant and safe physical environment.
This checklist will help hospital facility professionals prepare for survey evaluations regarding emergency preparedness requirements from the Centers for Medicare & Medicaid Services (CMS). Similar to the K-Tags CMS uses for life safety, the series of E-Tags developed by CMS provide interpretive guidance for emergency preparedness requirements

ASHE’s K-Tag Preparedness Survey Readiness Checklist and Crosswalk provides a comparison of the life safety and health care facilities regulations applicable to most hospitals. This resource represents CMS form 2786R Fire Safety Survey Report 2012 Code – Health Care, available at the time of this publication (September 2022) along with the applicable codes and standards references, accrediting organization references and available ASHE resources regarding the specific K-tag. It is important to verify the editions of the codes and standards that are applicable in your jurisdiction. Some jurisdictions may have adopted a different edition of the building code for construction. Many states have adopted other editions of the NFPA’s Life Safety Code®. For additional information, contact your state agency responsible for licensing hospitals.
